June 17, 2013 – June 23, 2013

PRELIMINARY INFORMATION: NO PURCHASE NECESSARY. A purchase will not improve your chances of winning. Void outside the WMAQ terrestrial geographic viewing area (as described below) and where prohibited. Live Nation Entertainment, Inc. Super Fan Contest (“Contest”) will begin on June 17, 2013 at 5:00 A.M. CT and end on June 23, 2013 at 11:59 P.M. CT (“Contest Period”). All times in the Contest refer to Central Time (“CT”). Odds of winning depend upon the number of eligible Entries (as defined below) received. Contest is subject to all applicable federal, state and local laws.

ELIGIBILITY: Open only to permanent, legal United States (“U.S.”) residents who are physically residing in the WMAQ terrestrial geographic viewing area (the counties of Cook, De Kalb, Du Page, Grundy, Kane, Kankakee, Kendall, Lake, La Salle, McHenry, and Will in the state of Illinois; and Jasper, Lake, La Porte, Newton, and Porter in the state of Indiana), and who are twenty one (21) years of age or older and of the age of majority in their state of residence  as of the start of the Contest Period. Officers, directors, and employees of Contest Entities (as defined below), members of these persons’ immediate families (spouses and/or parents, children, and siblings, and their spouses, regardless of where they reside), and/or persons living in the same households as these persons (whether or not related thereto) are not eligible to enter or win the Contest. Contest Entities, as referenced herein, shall include WMAQ, 454 N Columbus Drive, Chicago, IL 60611, NBCUniversal Media, LLC, 30 Rockefeller Plaza, New York, NY 10112 (collectively, “Sponsors”), Live Nation Entertainment, Inc. (“Prize Provider”), and their respective parent, subsidiary, and affiliate companies, and their administrative, advertising, and promotion agencies, and any other entity involved in the development, administration, promotion, or implementation of the Contest.

TO ENTER: To enter the Contest, during the Contest Period, visit WMAQ’s Facebook page located at http://www.facebook.com/nbcchicago (the “Website”) and click the “Like” button and becoming a fan WMAQ on Facebook. Then, access the Contest tab, and follow the provided instructions to complete and thereafter submit the entry form, which includes your name, email address, and date of birth (“Entry”). As a part of your Entry, you will be required to submit one (1) video that fits the following theme: “Why are you a Super Fan?” (the “Video”). You must submit the Video online according to the provided instructions and attach the Video to an email (“Email”), addressed to superfanconcertseries@gmail.com, with the subject line “Live Nation Entertainment, Inc. Super Fan Contest”. In addition to the Video, your Email must contain your name, email address, and date of birth. Video must be in one (1) of the following electronic formats: MPEG (.mpg), QuickTime (.mov), AVI (.avi), Windows Media (.wmv), Flash Video (.flv), and Real Media (.rm). Video must be no larger than one hundred (100) MB in size and no longer than sixty (60) seconds in length.  If Video exceeds sixty (60) seconds in length, only the first sixty (60) seconds will be judged. Video cannot have been created professionally and must not contain watermarks or other copyright notices. Your Video must be original (that is, filmed, created, and edited by you), true, and verifiable. Video must not contain any material that infringes upon any personal, proprietary, or any other right of any third party. You must have the permission of any person that you feature, identify, or otherwise refer to in your Video, and Video must not contain the name or likeness of any person, living or dead, unless such person has consented to such use. If any minors appear in your Video, you must be that minor’s parent and/or legal guardian. Other than minor changes to lighting and coloring, Video must not have been altered or edited in any manner that distorts the original Video. If you are already a fan of the Website, visit the Website, click on the Contest tab and then follow the provided instructions to complete and thereafter submit your Entry and Video. All Entries become the property of Sponsors and will not be acknowledged or returned.

You must submit the Video as part of your Entry to enter and be eligible for the Contest. By entering the Contest, entrants grant Sponsors the non-exclusive, royalty-free, and irrevocable rights to use, reproduce, copy, publish, display, distribute, perform, translate, adapt, modify, and otherwise exploit the Video and to incorporate the Video in other works in any and all markets and media now or hereafter known, throughout the world in perpetuity, without compensation, notification, permission, or approval. Entrants warrant that they have the sole and exclusive right to grant such rights to Sponsors and that the Sponsors' reproduction, publishing, displaying, and/or other use of the Video will not infringe on any rights of third parties, including, without limitation, copyright, trademark, privacy, or publicity, or create claims for defamation, false light, idea misappropriation, intentional or negligent infliction of emotional distress, or breach of contract. If any Video contains material that is violent, pornographic, obscene, illegal, inappropriate, or racially or morally offensive or if any Video does not comply with these Official Rules or meet Sponsors’ standards for any reason, as determined by Sponsors in their sole discretion, such Video (and the related Entry) may be rejected as ineligible for consideration. Entries must comply with all applicable laws, rules, and regulations. Your Video and any other portion of the Entry may be broadcast by Sponsors and published on www.nbcchicago.com. Sponsors, however, shall have no obligation to copy, publish, display, or otherwise exploit the Video.

Entries must be received before June 23, 2013 at 11:59 P.M. CT to be eligible for the Contest. Sponsors’ computer shall be the official timekeeper for all matters related to this Contest. You may enter once during the Contest Period. Multiple Entries received from any person or email address beyond this limit will void all such additional Entries. Entries generated by a script, macro, or other automated means will be disqualified. Entries that are incomplete, garbled, corrupted, or unintelligible for any reason, including, but not limited to, computer or network malfunction or congestion, are void and will not be accepted. In case of a dispute over the identity of an entrant, the authorized account holder of the email address used to enter will be deemed to be the entrant. “Authorized account holder” is defined as the person who is assigned to the email address by an Internet access provider, online service provider, or other organization that is responsible for assigning email addresses for the domain associated with the submitted email address. Entry constitutes permission (except where prohibited by law) to use entrant’s name, city, state, likeness, image, and/or voice for purposes of advertising, promotion, and publicity in any and all markets and media now or hereafter known, throughout the world in perpetuity, without additional compensation, notification, permission, or approval.

PRIVACY: Entrants will have the opportunity to receive information from WMAQ and selected partners by checking the appropriate box(es). If, at any time, you no longer wish to receive materials from WMAQ or its partners, please go to its privacy policy, located at http://www.nbcchicago.com/privacy/, or the applicable partner’s privacy policy, and follow the procedures indicated.

WINNER SELECTION AND NOTIFICATION: On or about June 26, 2013, one (1) or more qualified judges selected by Sponsors (“Judges”) will review the eligible Entries received during the Contest Period and select one (1) potential winner (“Winner”) based on the following judging criteria (“Judging Criteria”): Originality (50%) and Creativity (50%). The entrant whose Entry receives the highest total score will be deemed to be the potential Winner. In the event of a tie, the Judges will break the tie by selecting the tied entrant who received the highest points from the Judges for Originality as potential Winner. In the event a tie remains, the tied Entries will be judged by an additional tie-breaking judge, using the Judging Criteria, to determine which of the tied entrants will be potential Winner. Decisions of Sponsors and Judges are final and binding with respect to all matters related to the Contest. Winning is subject to verification of eligibility. Sponsors will make two (2) attempts to notify potential Winner at the phone number and/or email address submitted at the time of entry. Potential Winner may be required to execute and return an affidavit of eligibility, release of liability, and, except where prohibited, publicity release (collectively, “Contest Documents”) within three (3) days of such notification. Noncompliance within this time period will result in disqualification, and, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria. Sponsors reserve the right to select no potential Winner, to cancel the Contest, or to extend the Contest Period, if an insufficient number of eligible, complete, tasteful, appropriate, or generally qualified Entries are received by Sponsors as determined by Sponsors in their sole discretion. If potential Winner cannot be reached, is found to be ineligible, cannot or does not comply with these Official Rules, or if Prize (defined below) or Prize notification is returned as undeliverable, potential Winner will be disqualified and time permitting, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria.

PRIZE: There will be one (1) prize awarded (“Prize”). Prize will consist of the following: two (2) tickets for Winner and one (1) guest (“Guest”) to the Counting Crows with The Wallflowers concert (the “Event”), scheduled to take place on July 7, 2013 at 7:00 P.M. CT at Charter One Pavilion, located at 1300 S. Lynn White Drive, Chicago, IL 60605. Prize also includes an opportunity for Winner and Guest to participate in a meet and greet (“Meet and Greet”) at the Event (subject to availability and the sole discretion of Prize Provider); an opportunity for Winner and Guest to attend the last twenty (20) minutes of the band’s sound check (“Sound Check”) (subject to availability and the sole discretion of Prize Provider); one (1) exclusive poster; one (1) thirty-dollar ($30) gift card to Counting Crow’s online merchandise store (subject to certain terms and conditions as specified by issuer); and one (1) VIP laminate. Winner and Guest must be able to attend the Event as determined by Sponsors in their sole discretion, or such Winner’s Prize may be forfeited, and time permitting, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria. Winner may also be asked to record or document experience at the Events by creating video recording at the Event, taking photographs at the Event, writing essay or blog posts about each Event, and/or interviewing attendees at each Event (“Event Participation”). At least one (1) representative of Sponsors and/or Prize Provider will attend the Event with Winner and assist with Event Participation. Prize does not include transportation or parking. Guest, if any, must be twenty-one (21) years of age or older.

Estimated Retail Value (“ERV”) of Prize is four-hundred and fifty dollars ($450). Actual Retail Value (“ARV”) of Prize may vary. Any difference between ARV and stated ERV will not be awarded. If ARV of Prize exceeds five hundred ninety nine dollars ($599), Winner will receive an Internal Revenue Service Form 1099 from Sponsors for the total ARV of each Prize for the calendar year in which such Prize was awarded. All details of Prize will be determined by Sponsors and/or Prize Provider in their sole discretion. Sponsors reserve the right to substitute a similar Prize (or Prize element) of comparable or greater value. All taxes and other expenses, costs, or fees associated with the acceptance and/or use of Prize are the sole responsibility of Winner. Prize cannot be transferred by Winner or redeemed for cash and is valid only for the items detailed above, with no substitution of Prize by Winner. If Prize is unclaimed within a reasonable time after notification from Sponsors, as determined by Sponsors in their sole discretion, it will be forfeited, and time permitting, an alternate Winner may be selected from the remaining eligible Entries based on the Judging Criteria at Sponsors’ sole discretion.

Sponsors shall have no responsibility or liability for cancellations, delays, or any other change by any company or person providing any element of Prize, and are not responsible or liable for any expenses incurred as a consequence thereof. Dates and/or times of the Event, Sound Check, and/or Meet and Greet are subject to change. Event, Sound Check and/or Meet and Greet are subject to cancellation. Winner and Guest agree to abide by all venue rules and regulations in connection with Prize. Event tickets are subject to certain terms and conditions as specified by issuer and thereupon. Location of seats, if any, at the Event will be determined by Sponsors in their sole discretion. Prize will be awarded “as is” with no warranty or guarantee, either express or implied by Sponsors. If Winner chooses to attend the Event, Sound Check, and/or Meet and Greet with no Guest, the remaining elements of Prize shall constitute full satisfaction of Sponsors’ Prize obligation to Winner and no additional compensation will be awarded. Guest, if any, may be required to execute and return releases of liability and, except where prohibited, publicity releases (collectively, “Guest Documents”), which must be returned with the Contest Documents, or Guest portion of Prize will be forfeited. Guest, if any, must be twenty-one (21) years of age or older.

EVENT PARTICIPATION: Sponsors, in their sole discretion, may choose to record and/or broadcast the appearances of Winner and/or Guest in connection with the Contest, Prize, and/or Event Participation. If Sponsors elect to incorporate any audio and/or video recording of the appearance of Winner and/or Guest in connection with the Contest, Prize, and/or Event Participation (the “Recording”) into television shows or any and all media now or hereafter known, the taping and/or the Recording will not be considered an employment opportunity and Winner and/or Guest will not be entitled to compensation or credit thereof. All details regarding the Recording are subject to Sponsors’ sole discretion. The content of the Recording as, and if, finally exhibited and the number of exhibits and re-airs thereof, if any, will be determined by Sponsors in their sole discretion. Winner and/or Guest will not have a right of review, notice, or approval of the content of the Recording or any exhibition or exploitation thereof. Sponsors expressly reserve the right to broadcast, feature, publish, exhibit, or exploit the Recording (or any edited version of the Recording) in their sole discretion in any and all markets and media worldwide in perpetuity without compensation, notification, permission, or approval. Sponsors shall have not obligation to broadcast, publish, display, or otherwise exploit the Recording.

Winner and/or Guest may be removed from the Event and/or disqualified from Contest, at the sole discretion of Sponsors, for any reason at any time, including but not limited to the advice of Contest Entities’ representatives at the Event (“Event Staff”) or an unauthorized disturbance, interruption, or unnecessarily rough physical or mental threat to the well-being of any other attendee at the Event or Event Staff by Winner and/or Guest. Any act performed by Winner and/or Guest defined by federal, state, or local statute or ordinance as constituting a criminal act may result in removal from the Event and/or disqualification from Contest. At the sole discretion of Sponsors, if Winner and/or Guest use profanity, or demonstrate any lewd or sexually suggestive gestures or full or partial nudity, the Sponsors shall have the right to remove the individual from the Event and/or disqualify Winner. At the sole discretion of Sponsors, Prize Provider, and Event Staff, if Winner and/or Guest create a potential for death, disability, injury, property damage or any other negative incident during the Event or during his/her acceptance and/or use (as applicable) of Prize, Sponsors shall have the right to disqualify Winner and to remove Winner and/or Guest from such Events. Disqualification and/or removal from the Event will result in forfeiture of Prize. At least one (1) representative of Sponsors and/or Prize Provider will attend the Event with Winner and assist with Event Participation. Event Participation may include but is not limited to the following: interviewing attendees about their experience at the Event, creating video recordings at the Event, taking photographs at the Event, and/or writing essays or blog posts about the Event.

PRIZE SUBMISSIONS: Sponsors and/or Prize Provider, in their sole discretion, may choose to record, exhibit, use and/or broadcast videos, photographs, blog posts, and/or any other submission created by Winner in connection with his/her use of or participation in the Prize (the “Prize Submissions”). If Sponsors and/or Prize Provider, in their sole discretion, elect to incorporate any Prize Submissions into www.nbcchicago.com (and other related websites), television shows, or any and all other media, the use will not be considered an employment opportunity, and Winner will not be entitled to compensation or credit thereof. Sponsors and/or Prize Provider will determine the nature of the use in their sole discretion. Sponsors and/or Prize Provider reserve the right to use and publish the Prize Submissions for marketing and publicity purposes, and Sponsors and/or Prize Provider may exploit the Prize Submissions howsoever they choose in any and all markets and media now known or hereafter devised, throughout the world in perpetuity, without additional notification, permission, approval, or compensation. If the Prize Submissions are broadcast or displayed, such broadcast or display will be at a time, for a length of time, and on a date determined by Sponsors and/or Prize Provider in their sole discretion. Winner will be required to return completed and signed appearance releases, confidentiality agreements and such other documents as may be required by Sponsors and/or Prize Provider. Failure of Winner to complete and execute these documents in a timely manner, as determined by Sponsors and/or Prize Provider in their sole discretion, may cause Prize to be forfeited. The content of the Prize Submissions as, and if, finally exhibited and the number of exhibits and re-airs thereof, if any, will be determined by Sponsors and/or Prize Provider in their sole discretion. Winner will not have a right of review, notice, or approval of the content of the Prize Submissions or any exhibition or exploitation thereof. Sponsors and/or Prize Provider have the right, in their sole discretion, to disqualify Winner, cause Prize to be forfeited, if Winner’s behavior is deemed by Sponsors and/or Prize Provider to be disruptive or if it may or does cause damage to person, property, or reputation of Sponsors and/or Prize Provider or if it otherwise violates the policies of Sponsors and/or Prize Provider, or if Winner does not follow the directions of Sponsors and/or Prize Provider at all times. These Official Rules and the Contest itself are neither a contract nor an application for employment.

ADDITIONAL TERMS: Sponsors reserve the right to permanently disqualify from any promotion any person they believe has intentionally violated these Official Rules.  Any attempt to deliberately damage the Contest or the operation thereof is unlawful and subject to legal action by Sponsors, who may seek damages to the fullest extent permitted by law. The failure of Sponsors to comply with any provision of these Official Rules due to an act of God, hurricane, war, fire, riot, earthquake, terrorism, act of public enemies, actions of governmental authorities outside of the control of Sponsors (excepting compliance with applicable codes and regulations), or other “force majeure” event will not be considered a breach of these Official Rules.  Released Parties assume no responsibility for any injury or damage to entrants’ or to any other person’s computer relating to or resulting from entering or downloading materials or software in connection with the Contest. Released Parties are not responsible for telecommunications, network, electronic, technical, or computer failures of any kind; for inaccurate transcription of entry information; for errors in any promotional or marketing materials or in these Official Rules; for any human or electronic error; or for entries that are stolen, misdirected, garbled, delayed, lost, late, damaged, or returned. Sponsors reserve the right to cancel, modify, or suspend the Contest or any element thereof (including, without limitation, these Official Rules) without notice in any manner and for any reason (including, without limitation, in the event of any unanticipated occurrence that is not fully addressed in these Official Rules).  In the event of cancellation, modification, or suspension, Sponsors reserve the right to select Winner from among all eligible, non-suspect entries received prior to the time of the event warranting such cancellation, modification, or suspension based on the Judging Criteria above. Notice of such cancellation, modification, or suspension will be posted at the Website. Sponsors may prohibit any entrant or potential entrant from participating in the Contest, if such entrant or potential entrant shows a disregard for these Official Rules; acts with an intent to annoy, abuse, threaten, or harass any other entrant, Sponsors, or Sponsors’ agents or representatives; or behaves in any other disruptive manner (as determined by Sponsors in their sole discretion). Sponsors reserve the right to modify these rules for clarification purposes without materially affecting the terms and conditions of the Contest.

DISPUTES: The Contest is governed by, and will be construed in accordance with, the laws of the State of New York, and the forum and venue for any dispute shall be in New York, New York. If THE controversy or claim is not otherwise resolved through direct discussions or mediation, it shall THEN be resolved by FINAL AND binding arbitration administered by JUDICIAL ARBITRATION AND MEDIATION SERVICES, INC., in accordance with its Streamlined Arbitration Rules and Procedures or subsequent versions thereof (“JAMS Rules”). The JAMS Rules for selection of an arbitrator shall be followed, except that the arbitrator shall be experienced and licensed to practice law in new york. All proceedings brought pursuant to this paragraph will be conducted in the County of new york. THE REMEDY FOR ANY CLAIM SHALL BE LIMITED TO ACTUAL DAMAGES, AND IN NO EVENT SHALL ANY PARTY BE ENTITLED TO RECOVER PUNITIVE, EXEMPLARY, CONSEQUENTIAL, OR INCIDENTAL DAMAGES, INCLUDING ATTORNEY’S FEES OR OTHER SUCH RELATED COSTS OF BRINGING A CLAIM, OR TO RESCIND THIS AGREEMENT OR SEEK INJUNCTIVE OR ANY OTHER EQUITABLE RELIEF.

Contest Entities, as referenced herein, shall include WMAQ, 454 N Columbus Drive, Chicago, IL 60611, Live Nation, 233 North Michigan Avenue # 2700, Chicago, IL 60601, and NBCUniversal Media, LLC, 30 Rockefeller Plaza, New York, NY 10112 (collectively, “Sponsors”), and their respective parent, subsidiary, and affiliate companies, and their administrative, advertising, and promotion agencies, and any other entity involved in the development, administration, promotion, or implementation of the Contest.

TO ENTER: To enter the Contest, during the Contest Period, visit WMAQ’s Facebook page located at http://www.facebook.com/nbcchicago (the “Website”) and click the “Like” button and becoming a fan WMAQ on Facebook. Then, access the Contest tab, and follow the provided instructions to complete and thereafter submit the entry form, which includes your name, email address, and date of birth (“Entry”). As a part of your Entry, you will be required to submit one (1) video that fits the following theme: “Why are you a Country Super Fan?” (the “Video”). You must submit the Video online according to the provided instructions and attach the Video to an email (“Email”), addressed to superfanconcertseries@gmail.com, with the subject line “Live Nation Super Country Fan Contest”. In addition to the Video, your Email must contain your name, email address, and date of birth. Video must be in one (1) of the following electronic formats: MPEG (.mpg), QuickTime (.mov), AVI (.avi), Windows Media (.wmv), Flash Video (.flv), and Real Media (.rm). Video must be no larger than one hundred (100) MB in size and no longer than sixty (60) seconds in length.  If Video exceeds sixty (60) seconds in length, only the first sixty (60) seconds will be judged. Video cannot have been created professionally and must not contain watermarks or other copyright notices. Your Video must be original (that is, filmed, created, and edited by you), true, and verifiable. Video must not contain any material that infringes upon any personal, proprietary, or any other right of any third party. You must have the permission of any person that you feature, identify, or otherwise refer to in your Video, and Video must not contain the name or likeness of any person, living or dead, unless such person has consented to such use. If any minors appear in your Video, you must be that minor’s parent and/or legal guardian. Other than minor changes to lighting and coloring, Video must not have been altered or edited in any manner that distorts the original Video. If you are already a fan of the Website, visit the Website, click on the Contest tab and then follow the provided instructions to complete and thereafter submit your Entry and Video. All Entries become the property of Sponsors and will not be acknowledged or returned.

You must submit the Video as part of your Entry to enter and be eligible for the Contest. By entering the Contest, entrants grant Sponsors the non-exclusive, royalty-free, and irrevocable rights to use, reproduce, copy, publish, display, distribute, perform, translate, adapt, modify, and otherwise exploit the Video and to incorporate the Video in other works in any and all markets and media now or hereafter known, throughout the world in perpetuity, without compensation, notification, permission, or approval. Entrants warrant that they have the sole and exclusive right to grant such rights to Sponsors and that the Sponsors' reproduction, publishing, displaying, and/or other use of the Video will not infringe on any rights of third parties, including, without limitation, copyright, trademark, privacy, or publicity, or create claims for defamation, false light, idea misappropriation, intentional or negligent infliction of emotional distress, or breach of contract. If any Video contains material that is violent, pornographic, obscene, illegal, inappropriate, or racially or morally offensive or if any Video does not comply with these Official Rules or meet Sponsors’ standards for any reason, as determined by Sponsors in their sole discretion, such Video (and the related Entry) may be rejected as ineligible for consideration. Entries must comply with all applicable laws, rules, and regulations. Sponsors shall have no obligation to copy, publish, display, or otherwise exploit the Video.

Entries must be received before May 5, 2013 at 11:59 P.M. CT to be eligible for the Contest. Sponsors’ computer shall be the official timekeeper for all matters related to this Contest. You may enter once during the Contest Period. Multiple Entries received from any person or Facebook account beyond this limit will void all such additional Entries. Entries generated by a script, macro, or other automated means will be disqualified. Entries that are incomplete, garbled, corrupted, or unintelligible for any reason, including, but not limited to, computer or network malfunction or congestion, are void and will not be accepted. In case of a dispute over the identity of an entrant, the authorized account holder of the Facebook account used to enter will be deemed to be the entrant. “Authorized account holder” is defined as the person who is assigned to the Facebook account and/or email address (as applicable) by an Internet access provider, online service provider, or other organization that is responsible for assigning Facebook accounts and/or email addresses (as applicable) for the domain associated with the submitted Facebook account and/or email address (as applicable). Entry constitutes permission (except where prohibited by law) to use entrant’s name, city, state, likeness, image, and/or voice for purposes of advertising, promotion, and publicity in any and all markets and media now or hereafter known, throughout the world in perpetuity, without additional compensation, notification, permission, or approval.

PRIVACY: Entrants will have the opportunity to receive information from WMAQ and selected partners by checking the appropriate box(es). If, at any time, you no longer wish to receive materials from WMAQ or its partners, please go to its privacy policy, located at http://www.nbcchicago.com/privacy/, or the applicable partner’s privacy policy, and follow the procedures indicated.

WINNER SELECTION AND NOTIFICATION: On or about May 6, 2013, one (1) or more qualified judges selected by Sponsors (“Judges”) will review the eligible Entries received during the Contest Period and select two (2) potential winners (“Winners”) based on the following judging criteria (“Judging Criteria”): Originality (50%) and Creativity (50%). Potential Winners will be the two (2) Entries that receive the highest total scores. In the event of a tie, the Judges will break the tie by selecting the tied entrant who received the highest points from the Judges for Originality as a potential Winner. In the event a tie remains, the tied Entries will be judged by an additional tie-breaking judge, using the Judging Criteria, to determine which of the tied entrants will be a potential Winner. Decisions of Sponsors and Judges are final and binding with respect to all matters related to the Contest. Winning is subject to verification of eligibility. Sponsors will make two (2) attempts to notify potential Winners at the phone number and/or email address submitted at the time of entry. Potential Winners may be required to execute and return an affidavit of eligibility, release of liability, and, except where prohibited, publicity release (collectively, “Contest Documents”) within three (3) days of such notification. Noncompliance within this time period will result in disqualification, and, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria. If a potential Winner cannot be reached, is found to be ineligible, cannot or does not comply with these Official Rules, or if a Prize (defined below) or Prize notification is returned as undeliverable, such potential Winner will be disqualified and time permitting, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria.

PRIZE: There will be two (2) prizes awarded (“Prizes”), with one (1) Prize to each Winner. Each Prize will consist of the following: two (2) tickets for Winner and one (1) guest (“Guest”) to four (4) different country music concerts from the following (selection subject to availability and the sole discretion of Sponsors): the Brad Paisley concert, scheduled to take place on May 11, 2013 at 7:00 P.M. CT at the First Midwest Bank Amphitheatre (the “Amphitheatre”), located at 19100 South Ridgeland Avenue, Tinley Park, IL 60477; the Tim McGraw concert, scheduled to take place on May 24, 2013 at 7:00 P.M. CT at the Amphitheatre; the Toby Keith concert, scheduled to take place on June 30, 2013 at 7:00 P.M. CT at the Amphitheatre; the Blake Shelton concert, scheduled to take place on August 3, 2013 at 7:00 P.M. CT at the Amphitheatre; the Miranda Lambert and Dierks Bentley concert, scheduled to take place on September 7, 2013 at 7:00 P.M. CT at the Amphitheatre; and the Rascal Flatts concert, scheduled to take place on August 17, 2013 at 7:00 P.M. CT at the Amphitheatre (each an “Event” and, collectively, the “Events”). Prize includes a VIP experience for Winner and Guest at the Events (details subject to availability and the sole discretion of Sponsors). Each Winner and Guest pair must be able to attend the four (4) Events as determined by Sponsors in their sole discretion, or such Winner’s Prize may be forfeited, and time permitting, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria. Winners may also be asked to record or document experience at Events by creating video recording at each Event, taking photographs at each Event, writing essay or blog posts about each Event, and/or interviewing attendees at each Event (“Event Participation”). At least one (1) representative of Sponsors will attend the Events with each Winner and assist with Event Participation. Prizes do not include transportation or parking.

Estimated Retail Value (“ERV”) of each Prize is five hundred and seventy-five dollars ($575). ERV of all Prizes is one thousand one hundred fifty dollars ($1,150). Actual Retail Value (“ARV”) of Prize may vary. Any difference between ARV and stated ERV will not be awarded. If ARV of each Prize exceeds five hundred ninety nine dollars ($599), each Winner will receive an Internal Revenue Service Form 1099 from Sponsors for the total ARV of each Prize for the calendar year in which such Prize was awarded. All details of Prizes will be determined by Sponsors in their sole discretion. Sponsors reserve the right to substitute a similar Prize (or Prize element) of comparable or greater value. All taxes and other expenses, costs, or fees associated with the acceptance and/or use of each Prize are the sole responsibility of each Winner. Prizes cannot be transferred by Winners or redeemed for cash and are valid only for the items detailed above, with no substitution of Prizes by Winners. If a Prize is unclaimed within a reasonable time after notification from Sponsors, as determined by Sponsors in their sole discretion, it will be forfeited, and time permitting, an alternate Winner may be selected from the remaining eligible Entries based on the Judging Criteria at Sponsors’ sole discretion. Sponsors shall have no responsibility or liability for cancellations, delays, or any other change by any company or person providing any element of Prizes, and are not responsible or liable for any expenses incurred as a consequence thereof. Dates and/or times of the Events are subject to change. Events are subject to cancellation. Winners and Guests agree to abide by all venue rules and regulations in connection with Prizes. Event tickets are subject to certain terms and conditions as specified by issuer and thereupon. Location of seats at the Events will be determined by Sponsors in their sole discretion. If a Winner chooses to attend any Event with no Guest, the remaining elements of such Winner’s Prize shall constitute full satisfaction of Sponsors’ Prize obligation to such Winner and no additional compensation will be awarded. Guests, if any, may be required to execute and return releases of liability and, except where prohibited, publicity releases (collectively, “Guest Documents”), which must be returned with the Contest Documents, or Guest portion of each Prize will be forfeited. If any Guest is a minor, Winners must be such minor’s parent or legal guardian and must execute and return the Guest Documents on such minor’s behalf.

EVENT PARTICIPATION: Winner may be removed from any Event and/or disqualified from Contest, at the sole discretion of Sponsors, for any reason at any time, including but not limited to the advice of Sponsors’ representatives at Events (“Event Staff”) or an unauthorized disturbance, interruption, or unnecessarily rough physical or mental threat to the well-being of any other attendee at the Events or Event Staff by Winner. Any act performed by Winner defined by federal, state of local statute or ordinance as constituting a criminal act may result in removal from the Events and/or disqualification from Contest. At the sole discretion of Sponsors, if Winner uses profanity, or demonstrates any lewd or sexually suggestive gestures or full or partial nudity, the Sponsors shall have the right to remove the individual from the Events and/or disqualify Winner. At the sole discretion of the Sponsors and Event Staff, if Winner creates a potential for death, disability, injury, property damage or any other negative incident during any Event or during his/her acceptance and/or use of Prize, the Sponsors shall have the right to disqualify Winner and to remove Winner from such Events. Disqualification and/or removal from Events will result in forfeiture of Prize. At least one (1) representative of Sponsors will attend Events with Winner and assist with Event Participation.

PRIZE SUBMISSIONS: Sponsors, in their sole discretion, may choose to record, exhibit, use and/or broadcast videos, photographs, blog posts, and/or any other submission created by each Winner in connection with his/her use of the Prize (the “Prize Submissions”). If Sponsors, in their sole discretion, elect to incorporate any Prize Submissions into the Website or other television shows, the use will not be considered an employment opportunity, and each Winner will not be entitled to compensation or credit thereof. Sponsors will determine the nature of the use in their sole discretion. If the Prize Submissions are broadcast or displayed, such broadcast or display will be at a time, for a length of time, and on a date determined by Sponsors in their sole discretion. Each Winner will be required to return completed and signed appearance releases, confidentiality agreements and such other documents as may be required by Sponsors. Failure of a Winner to complete and execute these documents in a timely manner, as determined by Sponsors in their sole discretion, may cause such Winner’s Prize to be forfeited. The content of the Prize Submissions as, and if, finally exhibited and the number of exhibits and re-airs thereof, if any, will be determined by Sponsors in their sole discretion. Winners will not have a right of review, notice, or approval of the content of the Prize Submissions or any exhibition or exploitation thereof. Sponsors have the right, in their sole discretion, to disqualify a Winner, cause the Prize to be forfeited, if any Winner’s behavior is deemed by Sponsors to be disruptive or if it may or does cause damage to person, property, or reputation of Sponsors or if it otherwise violates the policies of Sponsors, or if a Winner does not follow the directions of Sponsors at all times. These Official Rules and the Contest itself are neither a contract nor an application for employment.

CONDITIONS: By entering the Contest, each entrant agrees for entrant and for entrant’s heirs, executors, and administrators (a) to release and hold harmless Contest Entities, Facebook, and their respective officers, directors, and employees (collectively, “Released Parties”) from any liability, illness, injury, death, loss, litigation, or damage that may occur, directly or indirectly, whether caused by negligence or not, from such entrant’s participation in the Contest and/or his/her acceptance, possession, use, or misuse of Prize or any portion thereof (including any travel related thereto); (b) to indemnify Released Parties from any and all liability resulting or arising from the Contest and to hereby acknowledge that Released Parties have neither made nor are in any manner responsible or liable for any warranty, representation, or guarantee, express or implied, in fact or in law, relative to Prize, including express warranties provided exclusively by Prize supplier that are sent along with Prize; (c) if selected as a Winner, to the posting of such entrant’s name on the Website and/or www.nbcchicago.com, and the use by Released Parties of such name, voice, image, and/or likeness and any photographs, videos, blog posts, and/or any other type submissions created by me in connection with my acceptance and use of the Prize for publicity, promotional, and advertising purposes in any and all media now or hereafter known, throughout the world in perpetuity, without additional compensation, notification, permission, or approval, and, upon request, to the giving of consent, in writing, to such use; and (d) to be bound by these Official Rules and to waive any right to claim any ambiguity or error therein or in the Contest itself, and to be bound by all decisions of the Sponsors, which are binding and final. If THE controversy or claim is not otherwise resolved through direct discussions or mediation, it shall THEN be resolved by FINAL AND binding arbitration administered by JUDICIAL ARBITRATION AND MEDIATION SERVICES, INC., in accordance with its Streamlined Arbitration Rules and Procedures or subsequent versions thereof (“JAMS Rules”). The JAMS Rules for selection of an arbitrator shall be followed, except that the arbitrator shall be experienced and licensed to practice law in new york. All proceedings brought pursuant to this paragraph will be conducted in the County of new york. THE REMEDY FOR ANY CLAIM SHALL BE LIMITED TO ACTUAL DAMAGES, AND IN NO EVENT SHALL ANY PARTY BE ENTITLED TO RECOVER PUNITIVE, EXEMPLARY, CONSEQUENTIAL, OR INCIDENTAL DAMAGES, INCLUDING ATTORNEY’S FEES OR OTHER SUCH RELATED COSTS OF BRINGING A CLAIM, OR TO RESCIND THIS AGREEMENT OR SEEK INJUNCTIVE OR ANY OTHER EQUITABLE RELIEF.

My name is Gabriela Piro and I'd like to formally enter my mom, Honorina (a.k.a. Nori) Vizcaino, the strongest, selfless mother I know for the "2013 Mother's Day Makeover Contest." Throughout my 36-years of life (I'm the oldest of three kids), my mom has always put her children/family first and foremost in her life. She spent the majority of her life raising us three crazy kids as a stay at home mom and taking care of her hub and for 38 years ... until his recent passing this past January.  My mom has always prioritized ensuring the family was happy and fulfilled way above and beyond her own self -- she made sure we had the "trendy" clothes, purses, toys, gimmicks and whatever else we cried and begged for. My mother barely ever splurged on herself and still today never treats herself to anything  -- she'll buy me a $80face cream from Bloomingdale's and by herself a $10 one from Target. And even though all three kids have long left the nest and are now married, she still can't seem to bring herself to splurge or concentrate on herself. She prefers to give onto others.


I think this make-over is the perfect kick-start to what my mom needs and deserves. After the recent passing of my father in January, its been very stressful on her and the rest of us as she learns to embrace her new future -- living on her own, managing her own finances/check book, and down-sizing her home. I can't imagine its easy on her but she seems to be pulling through this like a champ. I would love nothing more than to give my mom this amazing makeover this coming Mother's Day. I'd love to finally "give back"!!!

My wife Amy deserves more than a makeover, she deserves a better life. My wife Amy is a midwife at St Mary's Hospital, a mom to our  2 year old son Cole, and a stepmom to my 10 year old Madison.

Amy has been an incredible mom to our son Cole but she's really stepped up to the plate helping me raise my 10 year old daughter Madison.  She delivers babies all the time, morning day and night, sacrificing a lot of time away her family so she can help new families bring new life into the world. Amy is the most unselfish I've met in my life.

She pushes forward getting calls in the middle of the night to run to the hospital , sometimes crying along the way, because of her exhaustion. Don't get me wrong, she loves what she does, but being a Midwife is extremely challenging. We've been trying to have another child this past year and had a misfortune with a miscarriage back in December at Christmas time.

This may have have been God's way of telling her to slow down. But I believe the stress that she carries, trying to make everyone else happy really takes a toll on her body. No one is more deserving than a little makeover and getaway (even if its just for a night) than my wife and Mom to our kids.

My mom may not wear a super-hero cape but she sure does deserve one. She takes care of 4 generations that live in our home; ages range from 7-90. She takes care of every single person and checks in with everybody every day. The 7 people that live in our home is her 90 year old father, her husband (my dad), 2 granddaughters (my nieces), my sister (who had 5 wrist surgeries in 2011) and then there is me. My name is Megan and I am 23 years old. I have had 22 orthopedic surgeries since 2006 in 4 different states. She has been there every step of the way and I do not know one other person that could do a better job of handling everything than her. She is constantly taking care of others and putting everyone else’s needs before her own. The way she carries herself makes everything she does look effortless; most of our family and friends do not realize what all she puts up with on a day-to-day basis. She is a true inspiration to us all.

When my mom got cancer in 2004, she told me, “Sometimes we can’t control what happens to our bodies so take control of the things you can.” That phrase has stuck in my head ever since. If we went about life feeling as though we do not have control over anything, it would make for a miserable life for sure. Her phrase has been my mantra ever since I got injured in 2005. My mom firmly believes for every negative, you have to offset it with a positive. Ex: you have to go to the doctor to get your staples removed so go out to lunch first. If she didn’t incorporate fun and humor around the house, I know we wouldn’t be able to get through the hard days.

My mom has a contagious, quirky spirit that makes anyone smile. There is something special about my mom that no matter where she goes, strangers always talk to her. It’s the weirdest thing. She can just be standing in line and all of a sudden random people start talking to her. She must give off a good aura. Music is our way of escaping the daily struggles of day to day life. My mom listens to all types of music. Whether it be my grandpa BLASTING Big Band music, my nieces playing their One Direction CD, or all of the random stations that she has set in her car. Currently her favorite song is Feel This Moment by Pitbull ft Christina Aguilera.

Without a doubt, my mom deserves to be Queen for the day. She is a wife, mother, grandmother, breast cancer survivor (celebrated 9 years remission in Jan.) and is the BEST caregiver anybody could ask for. This year in particular, is going to be a very challenging, adventurous year. She will be taking me for medical treatment to Colorado which is crazy expensive! Insurance companies don't cover airfare, food, lodging etc. She and I have been to CO 3, soon to be 4 times since October 2012. Since my shoulder injuries are so complicated and severe, my doctor wants me to live in CO for physical therapy for 2-3 months. My mom will be there EVERY step of the way for me. She is a “Mama Bear” who protects, supports, and loves with all her heart.

My mom really gives everything her all; PLEASE CHOOSE HER! She deserves something special!!

Ever since I can remember I’ve been trying to convince everyone around me that my eyes are hazel.  For just as long, my mom has told me that she was so grateful that your brown eyes were passed on to me, because the rest of her children had blue eyes.  Even though I’ve resented it a little bit, making everyone else confess that I have a little bit of green in them, I’ve realized what a blessing having my mother’s bright brown eyes really is.

Through these brown eyes, my mom sees the world in such a different way from everyone else.  I’ve never come in contact with another woman who sees such positivity and good in everyone she encounters as my mother does.  No matter the situation, she always has a way of brightening it; she brings joy, happiness, and an overall sense of relaxation and relief to everyone around her.

For someone with an easy life, this positive attitude would be a small task.  My mother has had anything but an easy life.  Mom has raised four of her own children, and three that she took care of after her former husband passed away, because their birth mother had already died of cancer.  Before my little sister and I were born, she was raising 5 children on her own, as a single parent.  After she met my dad, having two more girls didn’t make it much easier.  That’s seven girls.  Seven.  At one time, there were five of us sharing a single bathroom.  It isn’t a stretch to imagine the MMA fights she had to break up about who got to use the shower, when.

Aside from just being a mother, she is a businesswoman.  For over 20 years, my mom owned and managed her own catering company in Evansville, Indiana out of our house.  I grew up in my version of heaven.  Everyday coming home from school, the smells of fresh baked bread permeated every room of the house.  It was magical.  Because of these childhood memories, and my admiration for my mother, I am currently in culinary school with hopes of becoming a caterer just like she was.

At times my mother was a single mom, a business owner, a fitness instructor, a cervical cancer survivor, a contender in the Miss Tennessee Pageant, in commercials for local businesses to bring income into the house, and a returning college graduate in her thirties.  All seven girls see my mom as the world’s greatest chef, the most positive person anyone will ever meet, a shoulder to cry on, someone who will always be more excited about your successes than you are even when it doesn’t seem possible, arms to hug, and the best listener.  More simply put, my mom is our hero.
 
The two youngest of the seven (myself included) are finally in college, and for the first time in over 30 years she doesn’t have anyone to take care of at home.  Being an empty nester is a new chapter in her life, and after fighting through cervical cancer, the struggles of being a single mom, her newly diagnosed rheumatoid arthritis, and the recent death of one of her most treasured people, her father (we all called him Pops), she deserves a treat.  She has spent her entire life caring for people, the least her daughters who love and appreciate her and look up to the beautiful, strong woman she is can do is try and win her this makeover.

 My mom has been trying to look for work for a few months now, and has so far been unsuccessful finding something that both suits her interests, and uses and challenges her talents.  I think giving my mom a makeover will not only bring her confidence when she’s looking for and interviewing for jobs, but also give her the spa day that she’s deserved for over thirty years now.
 
My mom is the best role model, mentor, caregiver, and friend any daughter could ask for.  My mom deserves this.  It is cliché to say that if it weren’t for her, I would be nowhere, but it’s true.  I am following her footsteps in becoming a caterer even though she has never once said anything to persuade me into this career.  I also plan on becoming a registered dietitian, and as a fitness instructor for decades, I think she had a big impact on my life that way too.  My mom is the most important person in the world to me; to all seven of her girls.  I love her so much, and I hope that as I grow older my brown eyes can see even half of the beauty the world as hers do everyday (especially if that includes a makeover from NBC!).

PRELIMINARY INFORMATION:  No purchase necessary. A purchase will not improve your chances of winning. Void outside the WMAQ Geographic Viewing Area (defined below) and where prohibited. The Live Nation Suit & Tie Contest (“Contest”) will begin on February 27, 2013 at 5:00 A.M. CT and end on March 1, 2013 at 5:00 P.M. CT (“Contest Period”). All times in the Contest refer to Central Time (“CT”). Odds of winning depend upon the number of eligible entries received.  Contest is subject all applicable federal, state and local laws.

In the event of a tie, the Judges will break the tie by selecting the tied entrant who received the highest points from the Judges for Originality as the potential Winner. In the event a tie remains, the tied entries will be judged by an additional tie-breaking judge, using the Judging Criteria, to determine which of the tied entrants will be the potential Winner. PRIZE: There will be one (1) prize awarded (“Prize”). Prize will consist of the following: two (2) tickets for Winner and one (1) guest (“Guest”) to Legends of Summer: Justin Timberlake and Jay-Z, scheduled to take place at Soldier Field, located at 1410 S. Museum Campus Drive, Chicago, IL 60605, on July 22, 2013 at 7:00 P.M. CT (“Show”). Prize does not include transportation or parking. Guest, if any, must be eighteen (18) years of age or older.

Estimated Retail Value (“ERV”) of Prize is three hundred and fifty dollars ($350). Actual Retail Value (“ARV”) of Prize may vary. Any difference between stated ERV and ARV will not be awarded. PRIZE: There will be one (1) prize awarded (“Prize”). The Live Nation Super Fan- Jason Mraz (“Contest”) will begin on August 31, 2012 at 9:00 AM CT and end on September 7, 2012 at 11:59 PM CT (“Contest Period”). All times in the Contest refer to Central Time (“CT”). Odds of winning depend upon the number of eligible Entries (as defined below) received. Contest is subject to all applicable federal, state and local laws.

TO ENTER: To enter the Contest, during the Contest Period, visit WMAQ’s Facebook page located at http://www.facebook.com/nbcchicago (the “Website”) and click the “Like” button and becoming a fan WMAQ on Facebook. Then, access the Contest tab, and follow the provided instructions to complete and thereafter submit the entry form, which includes your name, email address and date of birth (“Entry”). As a part of your Entry, you will be required to submit a video that fits the following theme: “Why are you a Jason Mraz Fan?” (“Video”). You may submit the Video online according to the provided instructions on the Website. Video must be in one of the following electronic formats: MPEG (.mpg), QuickTime (.mov), AVI (.avi), Windows Media (.wmv), Flash Video (.flv), and Real Media (.rm). Video must be no larger than one hundred (100) MB in size and no longer than sixty (60) seconds in length.  If Video exceeds sixty (60) seconds in length, only the first sixty (60) seconds will be judged. Video cannot have been created professionally. All Entries become the property of Sponsors, and will not be acknowledged.

You must submit the Video as part of your Entry to enter and be eligible for the Contest. You must have the permission of any person that you feature, identify or otherwise refer to in your Video. By entering the Contest, entrants grant Sponsors the non-exclusive, royalty-free, and irrevocable rights to use, reproduce, copy, publish, display, distribute, perform, translate, adapt, modify, and otherwise exploit the Video and to incorporate the Video in other works in any and all markets and media worldwide in perpetuity.  Entrants warrant that they have the sole and exclusive right to grant such rights to Sponsors and that the Sponsors' reproduction, publishing, displaying, and/or other use of the Video will not infringe on any rights of third parties, including, without limitation, copyright, trademark, privacy, or publicity, or create claims for defamation, false light, idea misappropriation, intentional or negligent infliction of emotional distress, or breach of contract.  If any Video contains material that is violent, pornographic, obscene, illegal, inappropriate, or racially or morally offensive or if any Video does not comply with these Official Rules or meet Sponsors’ standards for any reason, as determined by Sponsors in their sole discretion, such Video (and the related Entries) may be rejected as ineligible for consideration. Entries must comply with all applicable laws, rules, and regulations.  Sponsors shall have no obligation to copy, publish, display, or otherwise exploit the Submissions.

Entries must be received before September 7, 2012 at 11:59 PM CT to be eligible for the Contest. Sponsors’ computer shall be the official timekeeper for all matters related to this Contest. You may enter once during the Contest Period. Multiple Entries received from any person or email address beyond this limit will void all such additional Entries. Entries generated by a script, macro, or other automated means will be disqualified. Entries that are incomplete, garbled, corrupted, or unintelligible for any reason, including, but not limited to, computer or network malfunction or congestion, are void and will not be accepted. In case of a dispute over the identity of an entrant, the authorized account holder of the email address used to enter will be deemed to be the entrant. “Authorized account holder” is defined as the person who is assigned to an email address by an Internet access provider, online service provider or other organization that is responsible for assigning email addresses for the domain associated with the submitted email address. Entry constitutes permission (except where prohibited by law) to use entrant’s name, city, state, likeness, image, and/or voice for purposes of advertising, promotion, and publicity in any and all media now or hereafter known, throughout the world in perpetuity, without additional compensation, notification, permission, or approval.

PRIVACY: Entrants will have the opportunity to receive information from WMAQ and selected partners by checking the appropriate box(es). If, at any time, you no longer wish to receive materials from WMAQ or its partners, please go to its privacy policy, located at http://www.nbcchicago.com/privacy/, or the applicable partner’s privacy policy, and follow the procedures indicated.

WINNER SELECTION AND NOTIFICATION: On or about September 9th, 2012, a panel of qualified judges selected by Sponsors (“Judges”) will review the eligible Entries received during the Contest Period and select a potential winner (“Winner”) based on the following judging criteria (“Judging Criteria”): Originality (50%) and Creativity (50%). Potential Winner will be the Entry that receives the highest total score. In the event of a tie, the Judges will break the tie by selecting the tied entrant who received the highest points from the Judges for Originality as the potential Winner. In the event a tie remains, the tied Entries will be judged by an additional tie-breaking judge, using the Judging Criteria, to determine which of the tied entrants will be the potential Winner. Decisions of Sponsors and Judges are final and binding with respect to all matters related to the Contest. Winning is subject to verification of eligibility. Sponsors will make two (2) attempts to notify potential Winner at the phone number and/or email address submitted at the time of entry. Potential Winner may be required to execute and return an affidavit of eligibility, release of liability, and, except where prohibited, publicity release (collectively, “Contest Documents”) within three (3) days of such notification. Noncompliance within this time period will result in disqualification, and, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria. If potential Winner cannot be reached, is found to be ineligible, cannot or does not comply with these Official Rules, or if prize or prize notification is returned as undeliverable, potential Winner will be disqualified and time permitting, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria.

PRIZE: There will be one (1) prize awarded (“Prize”) to Winner. Prize will consist of a VIP experience for Winner and up to one (1) guest (“Guest”) to Jason Mraz on September 15, 2012 at 7:00 PM CT at Charter One Pavilion at Northerly Island, 1300 South Lynn White Drive, Chicago, IL 60605 (“Event”). Prize includes: two (2) tickets for Winner and Guest to attend the Event; and access into the Sound Check Party. Winner may also be asked to record or document experience at Event by creating video recording at Event, taking photographs at Event, writing essay or blog posts about Event, and/or interviewing attendees at Event (“Event Participation”). At least one (1) representative of Sponsors will attend Event with Winner and assist with Event Participation. 

Estimated Retail Value (“ERV”) of Prize is one hundred thirty dollars ($130). Actual Retail Value (“ARV”) of Prize may vary. Any difference between ARV and ERV will not be awarded. All details of Prize will be determined by Sponsors in their sole discretion. Sponsors reserve the right to substitute a similar Prize (or Prize element) of comparable or greater value. All taxes and other expenses, costs, or fees associated with the acceptance and/or use of Prize are the sole responsibility of Prize Winner. Sponsors will furnish to Winner an Internal Revenue Service Form 1099 for the ARV of Prize in the tax year it was awarded. Prize cannot be transferred by Winner or redeemed for cash and is valid only for the items detailed above, with no substitution of Prize by Winner. Event tickets are subject to certain terms and conditions as specified by issuer and thereupon. Location of seats at Event will be determined by Sponsors in their sole discretion. Date and time of Event are subject to change. Event is subject to cancellation. Winner and Guest agree to abide by all venue rules and regulations in connection with Prize. If Prize is unclaimed within a reasonable time after notification from Sponsors, as determined by Sponsors in their sole discretion, it will be forfeited, and time permitting, an alternate Winner may be selected from the remaining eligible Entries at Sponsors’ sole discretion.

EVENT PARTICIPATION: Winner may be removed from Event and/or disqualified from Contest, at the sole discretion of Sponsors, for any reason at any time, including but not limited to the advice of Sponsors’ representatives at Event (“Event Staff”), by creating an unauthorized disturbance, interruption, or unnecessarily rough physical or mental threat to the well-being of any other attendee at Event or Event Staff, he/she may be removed from the Event. Any act performed by Winner defined by Federal, State of Local statute or ordinance as constituting a criminal act may result in removal from Event and/or disqualification from Contest. At the sole discretion of Sponsors, if Winner uses profanity, or demonstrates any lewd or sexually suggestive gestures or full or partial nudity, the Sponsors shall have the right to remove the individual from Event and/or disqualify Winner. At the sole discretion of the Sponsors and Event Staff, if Winner creates a potential for death, disability, injury, property damage or any other negative incident during Event or during his/her acceptance and/or use of Prize, the Sponsors shall have the right to disqualify Winner and to remove Winner from Event. And disqualification and/or removal from Event will result in forfeiture of Prize. At least one (1) representative of Sponsors will attend Event with Winner and assist with Event Participation. 

RECORDING: Sponsors, in their sole discretion, may choose to record and/or broadcast videos, photographs, blog posts, and/or any other submission created by Winner in connection with his/her use of the Prize (“Prize Submissions”). If Sponsors, in their sole discretion, elect to incorporate any Prize Submissions into the Website or other television shows, the use will not be considered an employment opportunity, and Winner will not be entitled to compensation or credit thereof. Sponsors will determine the nature of the use in their sole discretion. If the Prize Submissions are broadcast, such broadcast will be at a time, for a length of time, and on a date determined by Sponsors in their sole discretion. Winner will be required to return completed and signed appearance releases, confidentiality agreements and such other documents as may be required by Sponsors. Failure of Winner to complete and execute these documents in a timely manner, as determined by Sponsors in their sole discretion, may cause the Prize to be forfeited. The content of the Prize Submissions as, and if, finally exhibited and the number of exhibits and re-airs thereof, if any, will be determined by Sponsors in their sole discretion. Winner will not have a right of review, notice, or approval of the content of the Prize Submissions or any exhibition or exploitation thereof. Sponsors have the right, in their sole discretion, to disqualify Winner, cause the Prize to be forfeited, if any Winner’s behavior is deemed by Sponsors to be disruptive or if it may or does cause damage to person, property, or reputation of Sponsors or if it otherwise violates the policies of Sponsors, or if Winners do not follow the directions of Sponsors at all times. These Official Rules and the Contest itself are neither a contract nor an application for employment.

CONDITIONS: By entering the Contest, each entrant agrees for entrant and for entrant’s heirs, executors, and administrators (a) to release and hold harmless Contest Entities, Facebook, and their respective officers, directors, and employees (collectively, “Released Parties”) from any liability, illness, injury, death, loss, litigation, or damage that may occur, directly or indirectly, whether caused by negligence or not, from such entrant’s participation in the or Contest and/or his/her acceptance, possession, use, or misuse of Prize or any portion thereof (including any travel related thereto); (b) to indemnify Released Parties from any and all liability resulting or arising from the Contest and to hereby acknowledge that Released Parties have neither made nor are in any manner responsible or liable for any warranty, representation, or guarantee, express or implied, in fact or in law, relative to Prize, including express warranties provided exclusively by Prize supplier that are sent along with Prize; (c) if selected as a Winner, to the posting of such entrant’s name on the Website and the use by Released Parties of such name, voice, image, and/or likeness and any photographs, videos, blog posts, and/or any other type submissions created by me in connection with my acceptance and use of the Prize for publicity, promotional, and advertising purposes in any and all media now or hereafter known, throughout the world in perpetuity, without additional compensation, notification, permission, or approval, and, upon request, to the giving of consent, in writing, to such use; and (d) to be bound by these Official Rules and to waive any right to claim any ambiguity or error therein or in the Contest itself, and to be bound by all decisions of the Sponsors, which are binding and final. Failure to comply with these conditions may result in disqualification from the Contest at Sponsors’ sole discretion.

ADDITIONAL TERMS: Sponsors reserve the right to permanently disqualify from any promotion any person they believe has intentionally violated these Official Rules.  Any attempt to deliberately damage the Contest or the operation thereof is unlawful and subject to legal action by Sponsors, who may seek damages to the fullest extent permitted by law. The failure of Sponsors to comply with any provision of these Official Rules due to an act of God, hurricane, war, fire, riot, earthquake, terrorism, act of public enemies, actions of governmental authorities outside of the control of Sponsors (excepting compliance with applicable codes and regulations), or other “force majeure” event will not be considered a breach of these Official Rules.  Released Parties assume no responsibility for any injury or damage to entrants’ or to any other person’s computer relating to or resulting from entering or downloading materials or software in connection with the Contest. Released Parties are not responsible for telecommunications, network, electronic, technical, or computer failures of any kind; for inaccurate transcription of entry information; for errors in any promotional or marketing materials or in these Official Rules; for any human or electronic error; or for entries that are stolen, misdirected, garbled, delayed, lost, late, damaged, or returned. Sponsors reserve the right to cancel, modify, or suspend the Contest or any element thereof (including, without limitation, these Official Rules) without notice in any manner and for any reason (including, without limitation, in the event of any unanticipated occurrence that is not fully addressed in these Official Rules).  In the event of cancellation, modification, or suspension, Sponsors reserve the right to select Winners from among all eligible, non-suspect entries received prior to the time of the event warranting such cancellation, modification, or suspension based on the Judging Criteria above.  Notice of such cancellation, modification, or suspension will be posted at Website.  Sponsors may prohibit any entrant or potential entrant from participating in the Contest, if such entrant or potential entrant shows a disregard for these Official Rules; acts with an intent to annoy, abuse, threaten, or harass any other entrant, Sponsors, or Sponsors’ agents or representatives; or behaves in any other disruptive manner (as determined by Sponsors in their sole discretion). 

DISPUTES: The Contest is governed by, and will be construed in accordance with, the laws of the State of New York, and the forum and venue for any dispute shall be in New York, New York. If THE controversy or claim is not otherwise resolved through direct discussions or mediation, it shall THEN be resolved by FINAL AND binding arbitration administered by JUDICIAL ARBITRATION AND MEDIATION SERVICES, INC., in accordance with its Streamlined Arbitration Rules and Procedures or subsequent versions thereof (“JAMS Rules”). The JAMS Rules for selection of an arbitrator shall be followed, except that the arbitrator shall be experienced and licensed to practice law in new york. All proceedings brought pursuant to this paragraph will be conducted in the County of new york. THE REMEDY FOR ANY CLAIM SHALL BE LIMITED TO ACTUAL DAMAGES, AND IN NO EVENT SHALL ANY PARTY BE ENTITLED TO RECOVER PUNITIVE, EXEMPLARY, CONSEQUENTIAL, OR INCIDENTAL DAMAGES, INCLUDING ATTORNEY’S FEES OR OTHER SUCH RELATED COSTS OF BRINGING A CLAIM, OR TO RESCIND THIS AGREEMENT OR SEEK INJUNCTIVE OR ANY OTHER EQUITABLE RELIEF.

All Entries become the property of Sponsors, and will not be acknowledged or returned. Entries must be received before August 17September 7, 2012  at 6:00 P.M. CT to be eligible for the Sweepstakes. Sponsors' computer shall be the official timekeeper for all matters related to this Sweepstakes. You may enter one (1) time per day during the Sweepstakes Period. Multiple Statements received from any person or /account beyond this limit will void all such additional Entries. For purposes of these Official Rules, a “day” will start at 12:00:01 AM CT and end at 11:59:59 PM CT. Entries that are incomplete, garbled, corrupted, or unintelligible for any reason, including, but not limited to, computer or network malfunction or congestion, are void and will not be accepted. WINNER SELECTION AND NOTIFICATION:   On or about September 778, 2012 , one fifteen (151) potential winners (“Winners”) will be selected in a random drawing from all eligible Entries received during the Sweepstakes Period by representatives of Sponsors. Sponsors will make two (2) attempts to notify potential Winner via direct message to the potential Winner's Twitter account in order to obtain potential Winner's mailing addresses. Potential Winner may be required to execute and return an affidavit of eligibility, release of liability, and, except where prohibited, publicity release (collectively, “Sweepstakes Documents”) within five (5) days of such notification.  Noncompliance within this time period may result in disqualification, and, at Sponsors’ sole discretion an alternate potential Winner may be selected from the remaining eligible Entries. PRIZE: There will thirty fifteen (1530) pPrize(s) awarded (“Prizes”) during the Sweepstakes, one (1) Prize to each Winner. Each Prize consists of the following: two (2) tickets for the Winner and up to one (1) guest (“Guest”) to attend one (1) Chicago Ideas Week Mega Talk  scheduled for October 8, 2012, October 9, 2012, orand October 11, 2012 at various locations in downtown Chicago (each an “Event”). Each Prize will be awarded “as is” with no warranty or guarantee, either express or implied by Sponsors. Sponsors reserve the right to substitute a similar Prize (or Prize element) of comparable or greater value at their sole discretion. All taxes and other expenses, costs, or fees associated with the acceptance and/or use of Prize are the sole responsibility of Winners. June 8, 2012—June 17, 2012

PRELIMINARY INFORMATION: NO PURCHASE NECESSARY. A purchase will not improve your chances of winning. Void where prohibited. Super Country Fan Contest (“Contest”) will begin on June 8, 2012 at 9:00 AM CT and end on June 17, 2012 at 11:59 PM CT (“Contest Period”). All times in the Contest refer to Central Time (“CT”). Odds of winning depend upon the number of eligible Entries (as defined below) received. Contest is subject to all applicable federal, state and local laws.

ELIGIBILITY: Open only to permanent, legal United States (“U.S.”) residents who are physically residing in the WMAQ terrestrial geographic viewing area (the counties of Cook, De Kalb, Du Page, Grundy, Kane, Kankakee, Kendall, Lake, La Salle, McHenry, and Will in the state of Illinois; and Jasper, Lake, La Porte, Newton, and Porter in the state of Indiana, and who are twenty one (21) years of age or older and of the age of majority in their state of residence  as of the start of the Contest Period. Officers, directors, and employees of Contest Entities (as defined below), members of these persons’ immediate families (spouses and/or parents, children, and siblings, and their spouses, regardless of where they reside), and/or persons living in the same households as these persons (whether or not related thereto) are not eligible to enter or win the Contest. TO ENTER: To enter the Contest, during the Contest Period, visit WMAQ’s Facebook page located at http://www.facebook.com/nbcchicago (the “Website”) and click the “Like” button and becoming a fan WMAQ on Facebook. Then, access the Contest tab, and follow the provided instructions to complete and thereafter submit the entry form, which includes your name, email address and date of birth (“Entry”). As a part of your Entry, you will be required to submit a video that fits the following theme: “Why are you a Country Super Fan?” (“Video”). You may submit the Video online according to the provided instructions on the Website. Video must be in one of the following electronic formats: MPEG (.mpg), QuickTime (.mov), AVI (.avi), Windows Media (.wmv), Flash Video (.flv), and Real Media (.rm). Video must be no larger than one hundred (100) MB in size and no longer than sixty (60) seconds in length.  If Video exceeds sixty (60) seconds in length, only the first sixty (60) seconds will be judged. Video cannot have been created professionally. All Entries become the property of Sponsors, and will not be acknowledged.

You must submit the Video as part of your Entry to enter and be eligible for the Contest. You must have the permission of any person that you feature, identify or otherwise refer to in your Video. By entering the Contest, entrants grant Sponsors the non-exclusive, royalty-free, and irrevocable rights to use, reproduce, copy, publish, display, distribute, perform, translate, adapt, modify, and otherwise exploit the Video and to incorporate the Video in other works in any and all markets and media worldwide in perpetuity.  Entrants warrant that they have the sole and exclusive right to grant such rights to Sponsors and that the Sponsors' reproduction, publishing, displaying, and/or other use of the Video will not infringe on any rights of third parties, including, without limitation, copyright, trademark, privacy, or publicity, or create claims for defamation, false light, idea misappropriation, intentional or negligent infliction of emotional distress, or breach of contract.  If any Video contains material that is violent, pornographic, obscene, illegal, inappropriate, or racially or morally offensive or if any Video does not comply with these Official Rules or meet Sponsors’ standards for any reason, as determined by Sponsors in their sole discretion, such Video (and the related Entries) may be rejected as ineligible for consideration. Entries must be received before June 17, 2012 at 11:59 PM CT to be eligible for the Contest. Sponsors’ computer shall be the official timekeeper for all matters related to this Contest. You may enter once during the Contest Period. Multiple Entries received from any person or email address beyond this limit will void all such additional Entries. Entries generated by a script, macro, or other automated means will be disqualified. Entries that are incomplete, garbled, corrupted, or unintelligible for any reason, including, but not limited to, computer or network malfunction or congestion, are void and will not be accepted. In case of a dispute over the identity of an entrant, the authorized account holder of the email address used to enter will be deemed to be the entrant. “Authorized account holder” is defined as the person who is assigned to an email address by an Internet access provider, online service provider or other organization that is responsible for assigning email addresses for the domain associated with the submitted email address. WINNER SELECTION AND NOTIFICATION: On or about June 18, 2012, a panel of qualified judges selected by Sponsors (“Judges”) will review the eligible Entries received during the Contest Period and select a potential winner (“Winner”) based on the following judging criteria (“Judging Criteria”): Originality (50%) and Creativity (50%). Potential Winner will be the Entry that receives the highest total score. In the event of a tie, the Judges will break the tie by selecting the tied entrant who received the highest points from the Judges for Originality as the potential Winner. In the event a tie remains, the tied Entries will be judged by an additional tie-breaking judge, using the Judging Criteria, to determine which of the tied entrants will be the potential Winner. Decisions of Sponsors and Judges are final and binding with respect to all matters related to the Contest. Winning is subject to verification of eligibility. Sponsors will make two (2) attempts to notify potential Winner at the phone number and/or email address submitted at the time of entry. Potential Winner may be required to execute and return an affidavit of eligibility, release of liability, and, except where prohibited, publicity release (collectively, “Contest Documents”) within three (3) days of such notification. Noncompliance within this time period will result in disqualification, and, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria. If potential Winner cannot be reached, is found to be ineligible, cannot or does not comply with these Official Rules, or if prize or prize notification is returned as undeliverable, potential Winner will be disqualified and time permitting, at Sponsors’ sole discretion, an alternate potential Winner may be selected from among all eligible remaining Entries based on the Judging Criteria.

PRIZE: There will be one (1) prize awarded (“Prize”) to Winner. Prize will consist of a VIP experience for Winner and up to one (1) guest (“Guest”) at four (4) different country music concerts: the Sugarland concert on June 23, 2012 at 7:00 PM CT at the First Midwest Bank Amphitheatre (the “Amphitheatre”), 19100 South Ridgeland Avenue, Tinley Park, IL 60477; the Rascal Flats concert on July 8, 2012 at 7:00 PM CT at the Amphitheatre; the Jason Aldean concert on August 18, 2012 at 7:30 PM CT at the Amphitheatre; and the Toby Keith concert on September 2, 2012 at 7:00 PM CT at the Amphitheatre (each an “Event” and, collectively, “Events”). Prize includes: two (2) tickets for Winner and Guest to attend each Event; free parking at each Event; a backstage tour for Winner and Guest at each Event; and BBQ dinner for Winner and Guest at each Event. Winner may also be asked to record or document experience at Events by creating video recording at each Event, taking photographs at each Event, writing essay or blog posts about each Event, and/or interviewing attendees at each Event (“Event Participation”). At least one (1) representative of Sponsors will attend Events with Winner and assist with Event Participation. 

Estimated Retail Value (“ERV”) of Prize is five hundred and sixty dollars ($560). Actual Retail Value (“ARV”) of Prize may vary. Any difference between ARV and ERV will not be awarded. All details of Prize will be determined by Sponsors in their sole discretion. Sponsors reserve the right to substitute a similar Prize (or Prize element) of comparable or greater value. EVENT PARTICIPATION: Winner may be removed from any Event and/or disqualified from Contest, at the sole discretion of Sponsors, for any reason at any time, including but not limited to the advice of Sponsors’ representatives at Events (“Event Staff”), by creating an unauthorized disturbance, interruption, or unnecessarily rough physical or mental threat to the well-being of any other attendee at Events or Event Staff, he/she may be removed from the Event. Any act performed by Winner defined by Federal, State of Local statute or ordinance as constituting a criminal act may result in removal from Events and/or disqualification from Contest. At the sole discretion of Sponsors, if Winner uses profanity, or demonstrates any lewd or sexually suggestive gestures or full or partial nudity, the Sponsors shall have the right to remove the individual from Events and/or disqualify Winner. At the sole discretion of the Sponsors and Event Staff, if Winner creates a potential for death, disability, injury, property damage or any other negative incident during any Event or during his/her acceptance and/or use of Prize, the Sponsors shall have the right to disqualify Winner and to remove Winner from such Events. And disqualification and/or removal from Events will result in forfeiture of Prize. At least one (1) representative of Sponsors will attend Events with Winner and assist with Event Participation. 

RECORDING: Sponsors, in their sole discretion, may choose to record and/or broadcast videos, photographs, blog posts, and/or any other submission created by Winner in connection with his/her use of the Prize (“Prize Submissions”). If Sponsors, in their sole discretion, elect to incorporate any Prize Submissions into the Website or other television shows, the use will not be considered an employment opportunity, and Winner will not be entitled to compensation or credit thereof. Sponsors will determine the nature of the use in their sole discretion. If the Prize Submissions are broadcast, such broadcast will be at a time, for a length of time, and on a date determined by Sponsors in their sole discretion. Winner will be required to return completed and signed appearance releases, confidentiality agreements and such other documents as may be required by Sponsors. Failure of Winner to complete and execute these documents in a timely manner, as determined by Sponsors in their sole discretion, may cause the Prize to be forfeited. The content of the Prize Submissions as, and if, finally exhibited and the number of exhibits and re-airs thereof, if any, will be determined by Sponsors in their sole discretion. ADDITIONAL TERMS: Sponsors reserve the right to permanently disqualify from any promotion any person they believe has intentionally violated these Official Rules.  Any attempt to deliberately damage the Contest or the operation thereof is unlawful and subject to legal action by Sponsors, who may seek damages to the fullest extent permitted by law. The failure of Sponsors to comply with any provision of these Official Rules due to an act of God, hurricane, war, fire, riot, earthquake, terrorism, act of public enemies, actions of governmental authorities outside of the control of Sponsors (excepting compliance with applicable codes and regulations), or other “force majeure” event will not be considered a breach of these Official Rules.  Released Parties assume no responsibility for any injury or damage to entrants’ or to any other person’s computer relating to or resulting from entering or downloading materials or software in connection with the Contest. Released Parties are not responsible for telecommunications, network, electronic, technical, or computer failures of any kind; for inaccurate transcription of entry information; for errors in any promotional or marketing materials or in these Official Rules; for any human or electronic error; or for entries that are stolen, misdirected, garbled, delayed, lost, late, damaged, or returned. Sponsors reserve the right to cancel, modify, or suspend the Contest or any element thereof (including, without limitation, these Official Rules) without notice in any manner and for any reason (including, without limitation, in the event of any unanticipated occurrence that is not fully addressed in these Official Rules).  In the event of cancellation, modification, or suspension, Sponsors reserve the right to select Winners from among all eligible, non-suspect entries received prior to the time of the event warranting such cancellation, modification, or suspension based on the Judging Criteria above.  Notice of such cancellation, modification, or suspension will be posted at Website.  Sponsors may prohibit any entrant or potential entrant from participating in the Contest, if such entrant or potential entrant shows a disregard for these Official Rules; acts with an intent to annoy, abuse, threaten, or harass any other entrant, Sponsors, or Sponsors’ agents or representatives; or behaves in any other disruptive manner (as determined by Sponsors in their sole discretion). 

DISPUTES: THE CONTEST IS GOVERNED BY, AND WILL BE CONSTRUED IN ACCORDANCE WITH, THE LAWS OF THE STATE OF NEW YORK, AND THE FORUM AND VENUE FOR ANY DISPUTE SHALL BE IN NEW YORK, NEW YORK. IF THE CONTROVERSY OR CLAIM IS NOT OTHERWISE RESOLVED THROUGH DIRECT DISCUSSIONS OR MEDIATION, IT SHALL THEN BE RESOLVED BY FINAL AND BINDING ARBITRATION ADMINISTERED BY JUDICIAL ARBITRATION AND MEDIATION SERVICES, INC., IN ACCORDANCE WITH ITS STREAMLINED ARBITRATION RULES AND PROCEDURES OR SUBSEQUENT VERSIONS THEREOF (“JAMS RULES”). THE JAMS RULES FOR SELECTION OF AN ARBITRATOR SHALL BE FOLLOWED, EXCEPT THAT THE ARBITRATOR SHALL BE EXPERIENCED AND LICENSED TO PRACTICE LAW IN NEW YORK. ALL PROCEEDINGS BROUGHT PURSUANT TO THIS PARAGRAPH WILL BE CONDUCTED IN THE COUNTY OF NEW YORK. THE REMEDY FOR ANY CLAIM SHALL BE LIMITED TO ACTUAL DAMAGES, AND IN NO EVENT SHALL ANY PARTY BE ENTITLED TO RECOVER PUNITIVE, EXEMPLARY, CONSEQUENTIAL, OR INCIDENTAL DAMAGES, INCLUDING ATTORNEY’S FEES OR OTHER SUCH RELATED COSTS OF BRINGING A CLAIM, OR TO RESCIND THIS AGREEMENT OR SEEK INJUNCTIVE OR ANY OTHER EQUITABLE RELIEF.
