As Toyota launches a recall that affects 3.8 million vehicles -- its largest ever -- the carmaker says it has pinpointed the cause of the problem: floor mats

The company says the mats can interfere with the gas pedal and cause it to get stuck. A stuck open accelerator pedal can result in high vehicle speeds and make it very difficult for a driver to stop the car.

Death and injury reports connected to the problem prompted Toyota to agree to the massive recall, but now a steady stream of voices from safety experts and from Toyota customers allege there is more than just floor mats to this problem.
