Health Agency Reports Breach of Private Information

More than 3,100 Cook County residents may be affected

An official with the Illinois Department of Healthcare and Family Services on Friday said personal information for more than 3,100 clients may have been released.

The agency said a contractor issued identification cards with clients' names, state-issued Medicaid numbers and birth dates and sent them to the wrong addresses. All of the clients are in Cook County, an official said.

IDHFS communication manager Kelly Jakuek said the agency was working with the contractor to identify all of the people affected and was taking "corrective measures," including issuing new cards.
