Do you know an ordinary person who does extraordinary things? Nominate them for the 2009 Jefferson Awards.
NBC5 Chicago will once again play homage to five selfless, committed individuals who work to better their communities through volunteer and community service.
Winners will be honored at the 14th annual Jefferson Awards ceremony held at the NBC5 studios in downtown Chicago.
If someone you know is working to make a difference in their community on behalf of others without expectations of recognition or reward, we encourage you to nominate them. Simply click here to download the form, complete and return it. Entries must be postmarked by February 13, 2009. (The form is a .pdf. You will need Adobe Reader to view it)
To be eligible, nominees must live in the Chicagoland area. The five Jefferson Award Honorees must be able to attend the awards ceremony. One of the Chicago honorees will be flown, all expenses paid, to the National Jefferson Awards in Washington, D.C. in June 2009.
The Jefferson Awards on NBC5 is part of the National Jefferson Awards, which is a program of the American Institute for Public Service. The goal of the institute is to inspire others to become involved in community and public services.
