A report by Chicago's inspector general indicates the city's police review agency has no record of investigating six shootings by officers in recent years.
The Chicago Tribune reports Inspector General Joseph Ferguson wrote that it was unclear if police failed to notify the Independent Police Review Authority of the shootings or if IPRA's record keeping was to blame. No one was injured in the shootings.
IPRA's head, Sharon Fairley, says the agency had no records indicating the department notified the agency of the six shootings, which took place between 2007 and 2011.
A department spokesman declined to comment Monday, but the mayor's office says the department will start issuing reports in September on use of force.
